Why do we hiccup? Medically, a hiccup is simple to describe: an involuntary spasm of the diaphragm followed a split-second later by the glottis — the opening between your vocal cords — snapping shut, producing the “hic.” What’s hard to explain is why this reflex exists at all. It serves no obvious purpose in humans. You can’t hiccup food down or air in.

The tadpole hypothesis

The most compelling explanation, advanced by respiratory researchers in the early 2000s, looks 370 million years back. Tadpoles and other amphibians at the gill-breathing stage use a two-part motor pattern: pump water into the mouth, then slam the glottis shut so the water exits across the gills instead of flooding the lungs. That sequence — sharp intake, immediate glottis closure — is neurologically identical to a hiccup.

The theory holds that the brainstem circuit for gill ventilation never got deleted as vertebrates moved ashore; it just got buried under newer breathing programs. Irritate the right nerves — a full stomach pressing the diaphragm, swallowed air, carbonation, sudden temperature changes — and the ancient subroutine fires again.

Why babies hiccup constantly

Supporting evidence: hiccups are most frequent exactly where evolution would predict — in the very young. Fetuses hiccup in the womb (mothers feel the rhythmic taps) beginning well before birth, and premature infants can spend a notable chunk of their day hiccuping. One hypothesis is that in nursing infants the glottis-slam has a real job again: keeping milk out of the lungs, like a burp with a security system. In adults, it’s probably pure legacy code.

The part most people miss

The medical record for longest hiccup attack belongs to Iowa farmer Charles Osborne, who hiccuped continuously from 1922 to 1990 — 68 years — while marrying twice and raising eight children. Almost every folk cure (breath holding, bag breathing, sugar swallowing, sudden fright) works by either raising blood CO2 or overriding the nerve loop — occasionally, and unreliably, which is why there are a hundred of them.
